The Woodruff Institute suffered from a data breach incident in June 2021.
The compromised information includes names, addresses, date of birth, telephone numbers (home and cell), health insurance information,type of test, and purpose.
The only SSNs we spotted were in older files where Medicare numbers appeared to be the individual’s SSN.
Additionally, there were a few files from 2018 that contained signed consents from patients to pay fees with recurring credit card payments.
In some circumstances, the form contains the complete credit card number information.
