Regina medical clinic failed to notify patients when a doctor’s dictation machine containing personal health information went missing a year ago.
One of the physicians and partners at the clinic, Dr. Noor Adams, had misplaced his dictaphone.
The dictaphone contained the names and diagnoses of 39 patients who Adams had seen at the clinic on Sept. 23, 2019.
