Faxton St. Luke's Healthcare

A data breach at Faxton St. Luke's Healthcare in New Hartford resulted in the exposure of 17,655 patients' personal information.

The information affected by the breach included patient names, dates of birth, prescription information, and, in some cases, the medical record number, according to the release.

Patients affected by the breach received letters through the mail and they monitored their free credit reports.
