Wisconsin Department of Health Services suffered a data breach incident that impacted the protected health information (PHI) of 12,358 individuals.
The exposed information included first and last name, date of birth, gender, county location, Wisconsin Medicaid member ID number, and social security number of affected members of Wisconsin Medicaid.
DHS immediately took safety precautions and notified the impacted individuals and offered them free credit monitoring services.
