Valley Anesthesiology and Pain Consultants (VAPC) suffred from a data breach incident that get 882,590 current and former patients and employee informations.
An individual had gained access to a system containing PHI.
The attack exposed the names, dates of service, health insurer name and ID number, diagnosis, treatment codes, and treatment locations, social Security and Medicare numbers.
They immediately investigated the incident and notified of the security incident by mail.
Individuals that have had their Social Security or Medicare number exposed, offered 12 months of credit monitoring and identity theft protection services without any charges.
