Triad Radiology Associates Reports Data Security Incident Affecting Patient Information
Triad Radiology Associates (TRA), a Winston-Salem, NC-based medical provider, has disclosed a data security incident involving unauthorized access to an employee email account. The breach may have exposed protected health information (PHI) of patients and individuals associated with other medical providers for which TRA provides radiology services.
Suspicious activity was first detected on or around July 30, 2025, with TRA confirming on December 10, 2025, that a limited set of personal data may have been accessible. Public notice was posted on TRA’s website on February 6, followed by direct notifications to affected individuals beginning February 11.
The compromised data varies by individual but may include names, addresses, Social Security numbers, driver’s license numbers, bank account details, dates of birth, medical records, and health insurance information. TRA stated there is no evidence the exposed data has been viewed or misused.
As part of its response, TRA is offering complimentary credit monitoring to impacted individuals and has notified the U.S. Department of Health and Human Services Office for Civil Rights. A dedicated hotline (1-800-405-6108) has been established for inquiries, operating Monday through Friday, 8 a.m. to 8 p.m. EST.
