Southern Illinois Healthcare suffered a data breach that exposed more than 600 patients' information.
The misdirected data includes names; addresses; Medicare numbers or Member ID numbers for non-Medicare patients.
They took immediate action to identify the cause and correct the error.
SIH mailed notice of the incident to affected patients and posted statements on its websites, sih.net and sihmedicalgroup.net.
SIH offered free credit repair and monitoring services to those potentially impacted.
