SOUTHWEST WASHINGTON REGIONAL SURGERY CENTER

Southwest Washington Regional Surgery Center in Vancouver suffered a data breach incident in September 2018.

The breach exposed some Protected Health Information, including some patients’ names, Social Security numbers, driver’s license numbers, and/or medical information (diagnosis, treatment, surgery, medications, labs and/or health insurance information) of 2,393 patients.

SWRSC offered a credit monitoring and identity theft restoration service to protect their information.
