Salt Lake Community College
A data breach occurred at Salt Lake Community College when it was discovered that 42000 students lost their tax records when an unencrypted drive slipped out of a mail envelope.
A memory drive containing the students' tax documents mysteriously dropped out of an envelope when it was being delivered from a contracted company to the institution, according to an email sent to the students that KSL Newsradio was able to receive.
It's still a tax document incident even though the types of tax documents implicated in this incident don't contain complete Social Security numbers like W-2 forms do.
