Primary Care Network

A Texas-based network of primary care clinics, Family Health Centers, suffered a healthcare data breach that permitted unauthorized access to some PHI.

The breach exposed information including the names, mailing addresses, Social Security numbers, dates of birth, and protected health information (PHI) of 233,948 individuals.

FMC started investigating the incident as soon as it discovered suspicious network activity.
