The massive number of unavailable or lost documents also includes “many incidents” where the files were eventually located.

Rather than ensconce patient data on encrypted platforms, the NHS often opts to use cheaper and more vulnerable means of recording , 94% of NHS Trusts still use handwritten notes for patient record keeping.
