Med Center Health suffered from a data breach incident that affected patients at The Medical Center Bowling Green.
The exposed information includes name, address, Social Security number, health insurance information, diagnosis and procedure codes, and charges for medical services.
However, the information obtained did not include patient medical records.
They notified patients as quickly as possible and offered free credit monitoring identity protection services for one year.
