Los Angeles County Department of Mental Health suffered a data breach incident in a phishing attack.

The compromised information included name, address, date of birth, driver’s license, Social Security number, medical and/or health information, health insurance information, SSID student identifier, and/or financial account number.

LACDMH investigated the incident with the help of a forensic firm and notified the impacted individuals.
