Little River Healthcare suffered from a data breach incident after that a briefcase contained18 paper charts and a laptop with patient electronic medical records, was stolen from a provider's locked vehicle.
The compromised information includes patient names, birth dates of service, and medical history.
The records did not include social security numbers, driver's licenses, or patient home addresses.
In order to avoid illegal access to personal accounts, Little River Healthcare has made its resources available to affected individuals and has urged consumers to get in touch with their financial institutions and credit bureaus.
