Jefferson Dental & Orthodontics

JDC Healthcare Management, LLC, also known as Jefferson Dental & Orthodontics, encountered a data security incident that compromised some user information after someone installed a malware program on its computer systems.

The compromised information includes parties’ Social Security numbers, passport numbers, driver’s license numbers, state identification numbers, dates of birth, clinical data, health insurance information, and financial information.

All individuals whose information was stolen started receiving data breach notification letters from JDC Healthcare Management.
