The breach was discovered after an unauthorized third party accessed files between December 12, 2025, and a subsequent investigation confirmed that compromised information included names, Social Security numbers, dates of birth, email and postal addresses, phone numbers, driver’s license and passport numbers, financial account details, medical records, and limited health-related data.
