Franciscan Physician Network

Franciscan Physician Network of Illinois and Specialty Physicians of Illinois suffered from a data breach incident that exposed 22,000 patient payment records.

The compromised information includes name, address, payment date, payment amount, payment method, office location and the last four digits of patient credit card numbers.

No full credit card number was compromised in the incident.

In addition to the patient's date of birth, the payment records from 2010 might also have included the facility's account number, the insurance ID number, the diagnosis, the type of visit, the procedure code, the provider's name and address, the dates of service, and the details of the services rendered.

The affected people have received a letter informing them of the situation, and they will be given two years of free identity theft protection services.

Also, concerned patients will be urged to take extra precautions by keeping an eye on their financial accounts, credit histories, and Explanation of Benefits documents.
