Eligibility & Compensation
Individuals affected by the breach may file claims for:
- Up to $5,000 in reimbursement for documented out-of-pocket losses (e.g., identity theft, credit monitoring fees, ID replacement costs).
- A one-time cash payment of approximately $90 (no documentation required).
- Two years of credit monitoring via CyEx Medical Shield Complete, including $1 million in medical identity theft insurance and fraud resolution support.
Claims must be submitted by June 29, 2026, either online or via mail. Payouts will be distributed after final court approval, expected by mid-2026.
Settlement Fund Allocation
The $2 million fund covers:
- Settlement administration costs.
- Up to $666,667 in attorneys’ fees.
- $15,000 in service awards for class representatives.
- Remaining funds for claimant payouts.
