Meadowlark Hills Data Breach Exposes Sensitive Information of Over 14,000 Individuals
The Manhattan Retirement Foundation, operating as Meadowlark Hills a nonprofit retirement community and nursing facility in Manhattan, Kansas recently disclosed a data breach affecting 14,442 individuals across the U.S., including one resident of Massachusetts. The incident involved unauthorized access to the organization’s network between July 12 and July 21, 2025, with the breach confirmed following an investigation that concluded on January 28, 2026.
The exposed data includes a range of sensitive personal and health information, such as full names, Social Security numbers, dates of birth, driver’s license or state ID numbers, financial account details, credit/debit card information, medical records, and health insurance policy data.
Meadowlark Hills filed disclosures with the Massachusetts Office of Consumer Affairs and Business Regulation and the U.S. Department of Health and Human Services. Written notifications to affected individuals began on or around February 26, 2026, with a public notice also posted on the organization’s website.
In response, Meadowlark Hills is offering complimentary single-bureau credit monitoring, credit reports, and credit score services for those whose Social Security numbers were compromised. Additional support includes fraud assistance and remediation services through Cyberscout, a TransUnion company. A dedicated toll-free response line (1-844-937-3566) has been established for affected individuals seeking further information.
