Circles of Care, Inc. suffered a data breach after an unauthorized party was able to access and steal confidential patient information from the organization’s computer network.
The unauthorized party gained access to consumers’ first and last names, dates of birth, Social Security numbers, addresses, phone numbers, driver’s license numbers, bank routing and account numbers, medical account numbers, provider names, service dates, diagnosis, and medical procedure codes.
Circles of Care sent out data breach notification letters to all individuals who were impacted by the recent data security incident.
