Child and Family Services (CFS) of New Hampshire suffered a physical data breach after a thief stole some files from the main Manchester, NH location.
The files included names, addresses, dates of birth, Medicaid numbers, and certain health information and notes from the home visits of 23 patients.
CFS immediately responded to the breach and alerted all the affected patients.
