CarePlus Health Plans is notifying 11,200 customers after a series of programming and printing errors resulted in explanation of benefit letters being sent to the incorrect members.

The breach was discovered on January 17.

The information disclosed included member name, CarePlus identification number and plan name,
Date of services ,Provider of service & Services provided.

The data was not been inappropriately used.
