Behavioral Health Partners of MetroWest LLC suffered a data security breach after an unauthorized party copied data from its computer system.
The incident compromised the personal information including name, address, Social Security number, date of birth, client identification number, health insurance information, and medical diagnosis or treatment information of about 11,288 individuals.
Behavioral Health Partners sent the breach notice to the affected individuals.
